Family Ways and our doulas are not participating / in-network providers with any insurance (including Tricare).
It is our clients' responsibility to confirm with insurance carriers whether reimbursement is available through insurance plans, and to submit documentation to insurance carriers.
We are not participating / in-network providers with any insurance. Additionally, as our doulas are not medical providers, we are unable to provide diagnostic (Dx) and procedure (CPT) codes; this may mean that your claim will be rejected. Itemized receipts cannot be provided until after services have been rendered. [Note: only our IBCLC may prepare a superbill, including Dx and CPT codes, for lactation services only.]
The fee below covers preparation of one set of documents (typically an itemized receipt) for submission for insurance. Please make sure to provide all necessary information (e.g., full legal name, current address, medical ID number, etc.) to help us prepare your receipt accurately. There is an additional $10 fee for any requested revisions to completed documentation.
